Admissions and Scheduling
- You will conduct a drug and alcohol pre-screen assessment with our admissions team.
- Our admissions team will verify any insurance coverage or discuss cash-pay options with you.
- You will be sent electronically ChoicePoint’s New Client Registration.
- You will be scheduled for your first appointment once the documentation is completed. Remote patients will be set up for their remote-drug testing compliance program.
- Medication insurance coverage is dependent on specific benefits. If a co-pay or any other patient responsibility for medications is required, please ensure to bring cash or a credit card for payment, as we are not allowed legally to pay for your medications.

- From here it will be determined the appropriate level of care and treatment program.
ChoicePoint offers different levels of care such as:
a. Outpatient/Ambulatory Detox with extended on-site medical monitoring.
b. Medication Assisted Treatment (Suboxone, Vivitrol)
c. Intensive Outpatient
d. Outpatient
e. Co-Occurring (Substance use alongside other mental-health items)

- Detoxes on average take 3-10 days and are entirely customized based on factors such as substance of use, amount, and duration of time.
- We recommend packing a lunch for each outpatient detox day, or just as common, clients will utilize delivery services such as DoorDash and Uber Eats.
- Outpatient detox days can range from around 4-6 hours a day depending on medical necessity.
- We recommend having transportation to and from the facility during these days, as a safety precaution.
Outpatient Detox
(If not entering detox level of care, skip to appropriate level of care)
- Detoxes on average take 3-10 days and are entirely customized based on factors such as substance of use, amount, and duration of time.
- We recommend packing a lunch for each outpatient detox day, or just as common, clients will utilize delivery services such as DoorDash and Uber Eats.
- Outpatient detox days can range from around 4-6 hours a day depending on medical necessity.
- We recommend having transportation to and from the facility during these days, as a safety precaution.
Medication-Assisted Treatment (MAT)
(In-Person or Virtual)
- On your first visit you will conduct a Nursing Assessment with our Nursing Team which will assess symptoms and assessment scores, and any necessary labs needed.
- You will then meet with our seasoned addiction Physicians and assess your medication-assisted treatment plan and order any necessary medications. Medications can be delivered to your home or picked up from your local pharmacy.
- You will meet with your Navigator who will help you set up appointments, recovery planning, and any additional support.
- You will meet weekly with your Physician for the first 4-6 weeks on average for Stabilization. Visits can last from 15 minutes-1 hour depending on severity or
Symptoms. - For the first 6 weeks you will also meet weekly with your Navigator who will continue to help you navigate your recovery journey. These usually average around 15 minutes a week.
- After you have been successfully stabilized on your MAT medication you will move into the maintenance phase.
- All MAT clients are required to participate in clinical services as part of their treatment to greatly increase your chances of long-term recovery compared to medication alone.

treatment plan, and averages three hours.
c. Support Sessions with those involved in your treatment to provide optimal
success.
- This level of care can last from around 30-90 days.
Intensive Outpatient
(In-Person or Virtual)
